The annual subscription for South Saxons Hockey Club is due on 1st September 2007.

Please complete a form (download here) and return it with your subscription to the Treasurer, Michelle Hartley.

As part of our Club 1st accreditation we are required to provide some statistics about our membership. As such we have added information to this year’s form and would ask that you please take the time to complete all the details. This is vital to us accessing future Sport England funding.

This information will be kept on an SSHC database and will only be used for internal administration and audit purposes and to keep you informed of Club activities.

Additionally to enable the management of Horntye to fulfil their obligations to Sport England information marked with a * will be shared with Horntye Park Sports Complex.

This information will not be sold or otherwise passed to any other third party.
